The HSE has written a formal apology to a woman with severe disabilities who allegedly suffered years of sexual and physical abuse in a foster care home in the south east.
It's claimed the woman remained in the home for 13 years after all other minors were removed and a warning was issued from authorities in the UK.
The HSE has said it already apologised to the victim - but this claim is disputed.
The Irish Examiner says the Health Service Executive has now written a formal apology - accepting that none of the failures should ever have happened.
